Aftercare Treatment

We believe aftercare and continuing care arrangements are critical to a patient's ongoing personal recovery.

Every patient who participates in chemical dependency and dual diagnosis treatment at the Life Center is required to complete a personal recovery plan with their case manager.

Plans are individualized, and based on the patient's progress towards meeting the goals and objectives identified on the Master Treatment Plan.

Ongoing involvment in a self-help program is considered essential to continuing recovery. Involvement in a professionally facilitated aftercare group either at the Life Center, or through the individual's community resources is also emphasized. Individual and family therapy on an outpatient basis is recommended as appropriate. Recommendations are also made for halfway house or supportive living environment when deemed necessary.

When halfway houses are recommended and the patient is agreeable, placement is secured prior to discharge from Life Center.
